24 hours notice for any appointment cancellations is required. If you do not attend your appointment and you have not formally cancelled, charges may be applied. This will not be charged in extenuating circumstances.

Please bring any forms with you to your appointment or have them dropped off or faxed to our office. Dr. Miller will complete this after clinic hours and you will be notified when it is complete. You may verify with your insurance company if you will be reimbursed for this fee. You will not be charged for a standardized post-operative work note.

Short/simple forms (less than 15 minutes to complete) - $20

Long/complex forms (longer than 15 minutes to complete) - $40-60 depending on length
